A gentle place to begin
Before forms, services or decisions, notice the small things: what is becoming harder, what is going well and what the person wants to keep doing.
The official pathway
My Aged Care describes four key stages for accessing Australian Government-funded aged care services.
Look at the types of help available and consider whether support at home, in the community or in an aged care home may suit your situation.
Apply for an assessment. The assessment helps My Aged Care understand your needs, goals and eligibility for services.
After assessment, compare providers in your area and consider the support, communication and routines that are important to you.
Choose a provider, discuss the details and agree on how services will work in your everyday life.
You do not need to solve everything at once. The first step can simply be noticing what would make an ordinary day feel easier.
Before an assessment
Assessments are about more than a list of tasks. It can help to think about the person's health, home, daily routines, current support and what they want to keep doing.
Make a note of activities that have changed, such as meals, mobility, shopping, housework or personal care.
Consider what matters most: staying connected, keeping a routine, feeling safe at home or building confidence.
A family member, friend, carer, advocate or other supporter may be able to be involved with the person's permission.
Canberra, in the detail
When you are considering support, the details of daily life are often the most important: the way someone likes their morning to begin, the people they see, the places they know and the routines that help them feel settled.
